FIRST VISIT

Chiropractic:

Upon entering the office, we have some paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition.

Functional Medicine:

Please contact us and we can email you your Comprehensive Health Intake Packet. Once filled out, we ask that you return it to us 48 hours prior to your initial appointment so that Dr. Wolfe may review your in depth health history. You may also include any recent labs or images with this packet for her to review.

CONSULTATION:

Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options.

Functional Medicine:

Expect to spend 1-1/2 hours at the office for your initial visit, including check-out time. You will spend 60 minutes with Dr. Wolfe. The consult fee includes time spent before and after your visit for Dr. Wolfe to review any past lab work and all paperwork and intake forms. Follow-up visits usually last 30-45 minutes depending on complexity and time to review lab work.

HISTORY AND EXAMINATION:

In order to determine what your actual problem is, Dr. Wolfe will ask you various questions related to your condition. A number of tests may be performed to determine the best course of treatment for you.

X-RAYS:

Chiropractic conditions may require an X-ray study to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

SAME DAY TREATMENT

Chiropractic- Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment on the same visit. This may include spinal adjustments, physical therapies, and/or soft tissue therapies.

SCHEDULE YOUR NEXT APPOINTMENT

You will be given a convenient appointment time for your follow-up visit. Generally speaking, chiropractic patients are seen within 1-2 days and are provided with a complete review of findings. Functional Medicine follow-ups are usually 2 weeks to a month apart. Treatment options for your condition will also be discussed during this time.
